Pelleve™ (pronounced “pelle-vay”) is a new, FDA-approved (May 2009) procedure for softening wrinkles on the face, neck, hands, (and potentially other areas of the body), and modestly “lifting” or “tightening” sagging skin in these same areas.  It may be considered a “mini-facelift” or “mini-filler” for age-wrinkles or sagging areas.

Pelleve™ can really be considered a very safe “youthfulness-producing” and “anti-aging” therapy.

How does it work?

Pelleve™ is a “touch” (non-surgical) medical procedure whereby radiofrequency (“RF”) waves, produced through a generator and delivered via a “wand” with a discoid tip applied to the skin travel  through the skin, targeting the “dermis” (the collagen-rich layer under the surface skin and above the fat) heating this layer and producing partial “denaturization” of the collagen.  Upon cooling immediately after application, the collagen reforms and “shrinks/stiffens” a bit, contracting the collagen layer, thereby “stretching/smoothing” wrinkles and tightening skin.

It is the collagen layer that gives the skin its support.  RF waves can easily travel through skin but cannot penetrate fat.  These electronic “waves” (not dissimilar to microwaves) become superheated in the dermis, producing the denaturization process.  Additionally, in the process, “micofractures” take place in the collagen, stimulating this layer, after 1-2 weeks, to produce a  build-up of new, strong collagen, further strengthening and supporting the skin.

What is a treatment like?

The procedure takes approximately 45 minutes (as long as an hour sometimes) per “area” (i.e. face, or neck” -which includes the décolletage area of chest, and hands if desired).  A conductive gel is applied to the treatment areas and RF waves travel into the dermis by means of a flattened ~ ½ inch applicator-wand.  There is a feeling of heat (like a hot rock treatment during massage).  Occasionally the skin momentarily becomes slightly hot, and treatment is directed to another area.  There are rare instances of slight “shock” (like walking on a nylon carpet and then touching someone.)  All-in-all it is not an uncomfortable experience.

How many treatments are necessary?  How long does it last?
For a younger person with only minimal wrinkles/sagging, only 1 treatment should be necessary.  For individuals with more pronounced changes, usually a series of 2-3 treatments spaced 4 weeks apart are necessary to achieve maximal effect.

Some results are seen immediately following the procedure.  After a few days, the initial stiffening subsides, and in a couple of weeks new collagen formation takes place.  Some results are immediate, although it takes 3-4 weeks to appreciate full results.

Facial results usually last a year, and up to a year and a half; neck/hand treatments last 1 ½ to 2 years. This is longer than other therapies available.

Am I a candidate for Pelleve™??

Pelleve™ is not a face lift. It does not “remove” wrinkles like Botox™, which is far more expensive in the long run, and requires re-injections every 3-6 months.  It is not a “filler” which is the only thing that can “remove” deep wrinkle grooves, but is expensive, and often-times produces poor results.

Pelleve™ is perfect for women and men who wish to look “less tired,” to soften the bags under their eyes and the folding of their upper eyelids, their “crows feet,” slightly “lift” their sagging cheeks or jowls, soften the wrinkles around their mouths and neck, tighten the “age-give-away” wrinkling and creasing on the back of their hands, etc.  It is truly a “rejuvenator.”  People will not notice the drastic changes of face lift or Botox™.  They (and you!!) will only notice that you look younger, less tired, and less stressed…

How does Pelleve™ compare with other “wrinkle therapies” such as Nu-Skin’s Age Loc(tm),” Thermage™, Laser, etc?

Age Loc™ produces similar results, but must be re-applied twice-weekly, each and every week, at home, to preserve results.  Although the initial expense is only ~
 $400-500, the gel necessary to continue therapy costs $40-$50/month, and the therapy is usually only used on the face.  Thermage™ uses pulsed, rather than continuous waves, is more uncomfortable, more expensive, and produces far less reliable results. This works well on some, but poorly on others.  Laser is actually a skin-resurfacing, rather than wrinkle- softening and tightening procedure.  Also, both Laser and Thermage are more expensive and mandate more “down-time” before an individual can return to normal public activities.  With Pelleve™ you can immediately put on your make-up (if used) and go back “out there”.  There is no residual pain or recovery period.  All that may be noticeable is a slight pinking of the skin for several hours.

What is the cost?  What about re- treatments?  Are there discounts?

Per-area (i.e. Face; Neck/hands) the cost is $750 for a single treatment, or $1500 for a “global fee” per areas, which includes 3 treatments, and is best for individuals with more than just minor skin changes.

Faculty member of the first Global Conference on Female Genital Cosmetic Surgery at the American Academy of Cosmetic Surgery meeting, Orlando, Florida, January 25, 2010. Dr. Goodman contributed to presentations and moderated discussion groups on ethical and patient protection issues in female genital plastic surgery as well as sexual issues in vaginal tightening procedures.

Attendance at and faculty member of the 30th annual North American Menopause Society meeting n San Diego, California in October of 2009. Dr. Goodman presented his poster presentation on the use of oral versus transdermal hormone therapy.

Faculty member at the annual International Society for the Study of Women's Sexual Health meeting in St. Petersburg, Florida, participating in the "Great Debate" on female sexual dysfunction and presenting a paper on his large nationwide female genital plastic surgery study.

Dr. Goodman was invited to give Grand Rounds at Sutter Hospital in Davis, Ca in October. His topic was "Transdermal (Bioidentical) Hormone Therapy".

The first chapter in a U. S. textbook on female genital aesthetic surgery, "Female Genital Plastic Surgery"  authored by Dr. Goodman, will be published in the new text, "Female Reproductive and Sexual Medicine" due out Fall, 2009.
